  • With over 25 years of experience, I provide expert evaluation and treatment for a wide range of neurologic conditions, including migraine, epilepsy, stroke, multiple sclerosis, neuromuscular and movement disorders (such as Parkinson’s), and dementias (including Alzheimer’s).

We are a fee-for-service practice. That means we do not bill your insurance. This allows us to take the time needed and remain patient focused, not disease focused. Our mandate is to provide thoughtful, comprehensive care by building connections and meeting the patient’s need, not insurance guidelines and checkboxes.

  • Most appointments available within 2 weeks and you communicate directly with Dr. Gajaweera. We aim to respond directly to all inquiries within 24 hours.

  • We understand that 9-5 does work for everyone. That’s why we have flexible appointment times. Let me know your preferences during our introductory call.

  • Home visits are so valuable. Whether it’s to accommodate a mobility impairment or to foster a patient’s ability to set the stage of their care, I truly enjoy the privilege of being invited in to see the place you call home.

  • While I do not participate or bill insurance I am able to provide you a “super-bill” which indicates the diagnostic and fee codes so that you can apply for out-of-network coverage. I am not able to guarantee or estimate out-of-network coverage in any way.

  • I do not participate in any insurances but most testing such as labs, imaging etc. are covered by your insurance just as they would be with any insurance participating provider. A few dementia preventive tests (genetics and amyloid/tau biomarkers) may not be covered by insurance but I am able to provide you with the costs for these through their vendor.

  • FSA and HSA funds can be applied in most instances. Please discuss this during our introductory call.

Directions: Located between CNY Fertility and the Stone-Tolan House, Building 2290 East Avenue shares a looped driveway with 2280 and 3000 off East Avenue. Enter from either direction, park and enter from the back of the building. Upon entering the foyer there is an elevator on the left. Take this to the 3rd floor and exit to the left. Pass a rest room on the right and just around the corner to the right you will find a set of stairs to the 4th floor landing and my office.
