This is for a 6 month comprehensive package and includes:
Three 90” in-person visits ((see prior page for details of package format)
Cognitive testing (using FDA-cleared Class II medical digital platform)
Review of any prior diagnostic testing
Interpretation and review of all testing ordered
Communication between visits regarding results
Individualized Dementia Prevention Strategy with detailed recommendations
Comprehensive written report of your results and recommendations along with long term goals to work on independently
I do not participate in any insurances, Medicare/Medicaid or any VA Medical services (including Tricare). By taking insurance out of the equation I am able to focus on what’s most important: your neurologic health.
Healthspan Neurology is NOT a “concierge” or subscription service; there is not a premium or monthly fee. My fee schedule is straightforward. You pay for when you are seen or my services are utilized and you will always be made aware of any anticipated fees. Payment is made at the time of scheduling of your visit.
Because I do not take insurance I am considered an “out of network” provider. You will be provided a receipt for all services after each visit and you can choose to apply for any out-of-network benefits or reimbursement from your insurance or HSA.
Coming soon: We will be partnering with Reimbursify to make it easy for patients to submit Superbills for out-of-network reimbursement.
